Transition Support for Youth

Dealing with behavioral health issues?

Young people learn, in a real world environment, to work together, encourage each other and prepare to be on their own.  We can help you make a successful transition to living independently. We work one-on-one to assist youth in gaining knowledge, skills, and connections.

We can work with you on: 

• Career planning
• Daily living skills
• Housing and money management
• Work life and work/study skills
• Communication
• Social relationships

Staff and youth work together to develop positive problem solving skills, self advocacy skills, positive peer relationships while addressing their educational and community living needs.
