Whole Person, Whole Health = Whole Community

What is Integrated Healthcare? It is the systematic coordination of integrating mental health, substance abuse, and primary care services when caring for people with multiple healthcare needs. People with mental and substance abuse disorders may die decades earlier than the average person — mostly from untreated and preventable chronic illnesses like hypertension, diabetes, obesity, and cardiovascular disease that are aggravated by poor health habits such as inadequate physical activity, poor nutrition, smoking, and substance abuse. Barriers to primary care, coupled with challenges in navigating complex healthcare systems have been a major obstacle to care.

At the same time, primary care settings have become the gateway to the behavioral health system, and primary care providers need support and resources to screen and treat individuals with behavioral and general healthcare needs.

We now offer Telehealth Appointments!

The health and safety of JFCS clients, staff and volunteers is our highest priority. It is our goal to continue to provide you services with as little interruption to your care as possible. In order to provide social distancing options, we are now offering our clients the ability to receive routine care from JFCS health professionals through a video connection on their smartphones in the comfort of their own home. These are also known as “telehealth appointments”.

JFCS can provide a virtual visit for the following services:
  • Intake and assessment
  • Psychiatric evaluation
  • Medication monitoring
  • Counseling

We accept all AHCCCS Health Plans, serving children, adolescents, families, adults and older adults. Please call 602-256-0528 to schedule your appointment.
